Will the M4V I get back be locked to a store or an account?

No, and the extension is what causes the confusion. M4V names a video file with the same internal structure as an ordinary MP4; the ones sold through a store are additionally wrapped in the seller's protection, and that wrapping is a property of the purchase rather than of the format. A file converted here has none of it — a plain, unprotected video that happens to carry that extension, playable wherever the extension is recognised and convertible onward as many times as you like. M4V is Apple’s variant of MP4, used by iTunes and the Apple TV app for films and TV episodes.
My MKV files came from ripping discs — will they convert cleanly?

Cleanly, yes, but check what is inside first. A rip usually carries more than the picture — often several audio languages, sometimes a subtitle track or chapter marks — and whether those survive depends entirely on what the M4V can hold, not on the rip. The other thing to know is that the encode already happened: the ripper chose a bitrate years ago and no conversion can recover detail it discarded, so if the discs are still around, re-ripping beats re-encoding. MKV is Matroska, an open container designed in 2002 with no licensing restrictions and no ceiling on what it can hold.
Both MKV and M4V are modern containers — which should I actually keep?
